injuries and accidents, overuse, family genetics, and inflammatory conditions. The term arthritis is also a general term that covers over 100 joint and musculoskeletal conditions.\n\nThe most common types are Osteoarthritis (OA, includes post-injury), Inflammatory arthritis (eg Rheumatoid arthritis), and Gout; a crystal type arthritis.\n\nIn the foot the most common site affected by arthritis is the 1st metatarso-phalangeal joint (MTPJ). Arthritis here is often called hallux limits or hallux rigidus. \n\nSymptoms of arthritis vary but can include;\naching and throbbing pain\nloss of movement\/joint stiffness\npain rest and at night\navoidance of the area\nswelling or enlargement of the joint\n\nXrays are used as a baseline investigation to help review the level\/stage of arthritic change within the foot, as well as bone stock\/quality. Other investigations may also be helpful including MRI or CT scan, and bloods to review inflammatory markers.\n\nTreatment options range from conservative treatments that may help to relieve the pain and discomfort associated with arthritis such as changing shoes, using orthotics, taking over-the-counter pain medications, or a course of injections for injections. Injections have evolved over the years where we can injection either cortisone\/steroid, hyaluronic acid, or plasma rich in growth factors (PRGF\/PRP). Many patients do well with conservative treatment however arthritis is usually a progressive problem requiring either self management repeat of the conservative options or surgery.\n\nSurgical treatment is recommended if you\u2019re suffering from severe pain from arthritis and simple measures have failed to improve your symptoms. Surgery will be specific to the joint(s) affected. Bunions can be painful, unsightly, and uncomfortable. If left untreated, they lead to footwear fitting problems, crowding and buckling of the other toes, arthritis within the bunion joint and increasing pain.\n \nA bunion is a shift of 2 bones - the 1st metatarsal and the base of the toe, where they form an offset angle and often a bony enlargement or bump.\n \nThe cause is thought to be genetic eg an inherited foot type. Years of wearing tight or uncomfortable shoes may exacerbate a bunion, but is not the initial cause.\n \nXrays are used to chart the degree of deformity and also help review any arthritic change within the foot, length of bones, bone stock\/quality and changes to other parts of the foot including the lesser toes and arch.\n \nConservative treatments may help to relieve the pain and discomfort associated with the bunion such as changing shoes, using orthotics, taking over-the-counter pain medications, or injections for pain relief eg cortisone\/steroid.\n \nIn some cases, surgery may be recommended as a treatment for bunions. The goal of surgery is to improve the alignment of the big toe joint. Commonly performed surgeries include osteotomy (moving and reshaping bones) or fusion (joining two bones). Depending on the extent of the deformity, additional procedures may be necessary.
